Re-registering and temporary patients

If you are a returning patient e.g. student, please make reception aware or add a comment in the comment box on the re-registration form of this fact so you can be re-registered and your new notes linked to your old notes.  This is important for both time and continuity of care.

Temporary registration is suitable for any patient wishing to access doctors services outside their own clinic.  Temporary residents will not be candidates for home visits but are welcome to attain advice from our team of doctors.

You must provide your NHS  number when registering with the practice. You can obtain this information from your previous GP surgery. To register with Parkbury House Surgery submit a registration form.

Once you have submitted your form, it will be passed onto the registration department. Please note, if there are any fields which have not been completed or your NHS number is not included this may delay your registration with us.

To complete the registration process you will need to come into the practice with photo ID and proof of address for each person registering with the practice. For children please bring in their birth certificate and red book/ immunisations record.
