New Patients

Parkbury House has an open list which means it accepts new patients to its Surgery.

The practice area includes the whole of St Albans, London Colney, Colney Heath, Sandridge, Chiswell Green, Smallford, and Park Street. Anyone living within this area is welcome to apply to register as a patient.

We cannot/do not accept new patients from: Harpenden, Redbourn, Wheathampstead, Colney Street, Frogmore, Shenley or Bricket Wood.

If you wish to become a patient at Parkbury House or wish to return to us after a period of travel or absence please complete a registration form at reception or complete and submit a form using the link below.

Re-registering and temporary patients

If you are a returning patient e.g. Student, please make reception aware or add a comment in the comment box on the re-registration form of this fact so you can be re-registered and your new notes linked to your old notes.  This is important for both time and continuity of care.

Temporary registration is suitable for any patient wishing to access doctors services outside their own clinic.  Temporary residents will not be candidates for home visits but are welcome to attain advice from our team of doctors.

You must provide your NHS  number when registering with the practice. You can obtain this information from your previous GP surgery. To register with Parkbury House Surgery please  submit a registration form.

Once you have submitted your form, it will be passed onto the registration department. Please note, if there are any fields which have not been completed or your NHS number is not included this may delay your registration with us.

Safe Surgery

Parkbury House is a ‘Safe Surgery’, meaning it commits to providing equal access to our services for everyone within our practice area, regardless of immigration status. This is in line with our duties under NHS Guidelines, and informed by our knowledge of the barriers to healthcare faced by migrants in vulnerable circumstances. Although we ask for ID and proof of address, if you do not have these documents but you live within our practice area, you can still register with us. We won’t ask for immigration documents.
